Improving Recovery by Getting Patients Home Sooner

Adam Carson, Managing Director of South Warwickshire NHS Foundation Trust, discusses initiatives to help people get out of the hospital more quickly. The focus is on the Home First agenda, recognising that hospital isn't always the best environment for recovery. Carson highlights the frailty service, which triages elderly patients and provides subacute intervention before they are admitted to a hospital bed. He also discusses the Community Recovery Service, which aims to rapidly discharge patients with care needs to their home or a domiciliary care provider. The collaboration between health and social care is crucial in making these initiatives successful.
